This movie started with Michael Kovak (Colin O'Donoghue) decided to enter a seminary school and abdicated his vows after he completed his free college degree. Four years passed and Michael was being ordained to the diaconate at the seminary. However, after ordination he decided to resign. In his resignation letter to Father Matthew, he claimed that he lack in faith. One rainy night, Father Matthew (Toby Jones) saw Michael on the road and wanted to talk Michael out of his decision. He attempted to catch up with Michael but tripped, causing a biker to swerve into the path of an oncoming car. She was knocked down by the car. Upon witnessing the accident, Michael went to help the young woman. The young woman, thought Michael was a priest and asked him for absolution. Unable to refuse, Michael performed the ritual to absolve her sins. Father Matthew saw the whole process and asked Michael to be a priest. Later, he approached Michael with an invitation to travel to Rome to attend a course on exorcism. Michael reluctantly accepted the offer. Father Matthew added that if he still insisted to resign from his position after taking the course, then he could leave.
During classes, he met a young woman, Angelina (Alice Braga), who was also taking the course. She was a reporter who was covering the course for an article in a newspaper. Father Xavier (Ciarán Hinds) was told that Michael's faith was shaken. He asked Michael to go see a friend of his, Father Lucas (Anthony Hopkins), who was a renowned Welsh Jesuit exorcist. Michael agreed and met Father Lucas at his home. While he was there, he saw one of the priest's patients: a pregnant sixteen-year-old girl. Later, he found out that she was raped by her father, which led to her being possessed. Even after witnessing several unexplained events, such as the girl coughing up three long nails and fluently spoke English, Michael still remained skeptical. Angelina who got to know that Michael frequently visit Father Lucas asked him to gave her information that he obtained from Father Lucas to her but Michael refused.
Meanwhile, the possessed girl's condition worsened. Father Lucas and Michael took her to a hospital for further care. There, Michael witnessed Father Lucas performed another exorcism on the girl. That night, Father Lucas stayed overnight outside the girl's room. Late that night, the girl suffered a miscarriage. Both the baby and the mother died. Feeling disheartened and disappointed with himself, Father Lucas felt that he had failed her.
After the death of the young woman, Father Lucas began to behave strangely. He displayed signs of demonic possession. One rainy day, Michael and Angelina found him sitting outside of his house in the rain. Father Lucas knew that he was possessed. He told Michael to find Father Xavier to perform an exorcism on him. Angelina and Michael tried desperately to contact Father Xavier but failed. He will be out of contact for 3 days. Out of desperation, Michael decided to perform the exorcism with Angelina present. After constant mocking by the demon and a hard fought battle with the demon, Michael regained his faith and forced the demon to reveal its name, which was Baal. He completed the exorcism and the demon freed Father Lucas. After his successful exorcism, Michael left Rome and went back to the states.
In the final scene, Father Michael Kovak, entered a confessional to hear a girl's confession, revealing that he is now a priest.
